Molis Coaching: Molis Moment — Pre-Restorative for Chipped Teeth

Often we do Invisalign to provide a straighter smile, but sometimes it actually becomes necessity as a treatment tool.

This is Richard, he came in today as a new patient. His chief complaint was two broken teeth. The front central is a little chipped. If you look at the bottom, that tooth is chipped, so he wanted those fixed.

If you look closer, you can see he has an edge to edge bite. In this situation, if I were to build up that corner with composite he would knock it off. If I did a veneer or a crown, he would chip it right off because there is no room.

We ended up doing an Invisalign scan to actually straighten out the teeth first before we fixed the chipped teeth. A great example of fixing an underlying problem for a much more impactful outcome!

Molis Coaching: Molis Moment — Placing Attachments on Crowns

Here is a case here where we had to place attachments on crowns. You will see in this case why it was a must.

I use a product from Pulpdent, called the Porcelain Prep Kit. You cannot use a normal etchant of bonding system because the attachments won’t stay on the crowns.

In Erica’s case, she has double attachments on 8 & 9, which are crowns. We had to do the double attachments to avoid tipping the teeth which would leave a black triangle, not to mention that the movement we need wouldn't happen anyway.

If your patient has crowns, use this Pulpdent Porcelain Etch system. Again, I might treat one out of every couple hundred patients this way -- it’s pretty rare -- but when it's a necessity you have to do it or the patient is not going to get the outcome that they want.

Molis Coaching: Molis Moment — Breaking a Thumb Sucking Habit

Let's go over a special case, not because of what it did for the patient's teeth but because of what it did for her habit.

Camille is an interesting case because we treated her to close an open bite, but she had a habit that was hard to break. She let us know that she has been thumb-sucking since she was a child and she is 28 years old now. She tried putting things on her finger to stop the thumb sucking, such as tabasco or bitter spray that did not taste good, nothing worked.

However! having the Invisalign in and not being able to suck her thumb helped her break the habit. The Invisalign created a barrier that made it uncomfortable for her to suck her thumb. She also wanted the results of Invisalign, which helped her stop.

Camille stopped her habit from the moment she started the trays. Definitely, within the first tray (2 weeks) she would go to brush her teeth but told herself no. Now after 22 trays she does not have the urge to suck her thumb at all.

Molis Coaching: Molis Moment — Appropriately Addressing TMJ Questions with Invisalign

TMJ is not something to be intimidated by. You're going to get asked about it from time to time. Noting that Invisalign is not a medical device designed to treat TMJ.

If you are asked if Invisalign treatment will solve a patient's TMJ, you can answer honestly and transparently: Moving teeth into a more proper position will reduce the stress on your TMJ. Fixing your bite can help alleviate the situation. You can't guarantee the end result.

Wearing the Invisalign retainer every night acts as a bite guard... and I believe it's the best bite guard on the market.

If we meet one of my patients, Heather, you'll see that she had severe crowding, a deep bite and more. We were able to straighten her teeth significantly and get her bite in great shape.

Molis Coaching: Molis Moment — Edge to Edge Bite Fix

Here's a really interesting result using precision cuts, hooks, and rubber bands.

The patient is Carla. She had 18 trays and an edge-to-edge bite. We requested precision cuts and hooks to bring the top 4 to the bottom back.

After 10 trays, Carla has been wearing rubber bands just about everyday.

What a difference wearing the rubber bands can make after just 10 trays! 

Her edge-to-edge bite is gone. Going forward, Carla only needs to wear the rubber bands at night.
